A leading Welsh charity is now looking for a Mental Health Support Worker to join the team as a part-time Domiciliary Care Planner and help coordinate specialist homecare for people in Pembrokeshire with a variety of complex needs (including physical disabilities, mental health conditions, learning disabilities, and frailty).

The charity provides comprehensive aid to vulnerable people with mental health, addiction & related conditions who are facing difficult circumstances, such as with substance misuse, the criminal justice system, or loss of sufficient housing/employment.

As a Domiciliary Care Planner, you will complete detailed care and risk assessments, create individualised, person-centred service delivery plans, and review existing plans to promote the health, wellbeing, rights, and independence of vulnerable people requiring help at home, whether they are new to the service or have evolving health and social care needs.

This is a permanent, part-time role (22.5h per week) for a Domiciliary Care Planner. There will be occasions where you may be required to work unsocial hours (including evenings and weekends) to support training, carry out observations, and cover any unplanned absences of the support workers to ensure continuity of service.

 

Person specification:

Candidates must have:

  • Previous experience in providing care and/or mental health support.
  • NVQ Level 2 /3 in Health & Social Care, or willingness to work towards.
  • Flexibility to work unsocial hours when required.
  • A valid UK driving licence for occasional work-related travel needs.
