Description: They are the NHS trust responsible for providing community health services for over 515,000 people living across the London boroughs of Hounslow and Richmond-upon-Thames. With over 1,000 staff. Community healthcare covers most of the services that are provided outside of hospital or GP surgeries.

The Community Recovery Service is an integrated health and social care service for adults identified with uni and multi-disciplinary recovery needs. The service will support independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances. It aims to restore, maximise or prevent deterioration in physical, psychological and social functioning through episodes of rehabilitation and reablement. The role in question requires a specialist clinician in a specific clinical area, participating as a member of the team independently managing a specialist clinical caseload.

Thus, you will provide high quality physiotherapy assessment and treatment to adults referred to the Community Recovery Service.

Person Specification: The applicant must be HCPC registered with a diploma/degree in physiotherapy or equivalent.

The following are essential: completed band 5 physiotherapy rotations and/or two years plus band 5 community Physiotherapy experience, broad range of rotational posts, broad experience in orthopaedics, medical, neurology and elderly, specific experience in speciality designated to post, and experience of working as part of a multidisciplinary team.

Desirable, the applicant should have knowledge of community and neuro-rehabilitation physiotherapy services, previous involvement in audit, research and quality issues, and have experience in supervision of junior staff.

Each post holder will have a set of skills and competencies associated with the safe delivery of assessment and therapeutic interventions relating to their specific post which may include but is not confined to:

* Neurorehabilitation (acute and long-term conditions)
* Falls
* Muculo-skeletal
* Respiratory
* Dementia
* Frail elderly
* Therapeutic modalities
* Amputees
* Bariatric
* Pain management
* Palliative care
