We have a fantastic opportunity for a Care Manager who has comprehensive complex care skills to work with care packages and make a difference to lives. You will deliver high standards of clinical and general care, play and activities while promoting a high quality service for the children and adults alike.

Our client provides support to both children and adults who have severe complex needs; each person will have a tailored comprehensive care plan which can often involve round the clock care; with some rare cases requiring full-time live in carers.

If successful you will be responsible for approximately 1,500 hours of care. You will need to supervise and support your team to maintain safe working practices for the clients within their portfolio, as well as being responsible for writing the rotas for each care package, carrying out spot checks, supervisions and being part of an on-call service for the staff and the clients on a rota basis.

Your job role will also include liaising with the CCGs, families and clinical leads to ensure that competent carers and nurses deliver excellent care.

It is essential that you drive and have experience in setting up care packages for adults and children with complex needs.

You must have access to your own vehicle.
