They have a number of exciting vacancies for Qualified District Nurses or very experienced community nurses in their Integrated Community Teams across the Borough. The District Nurse/Senior Community Staff Nurse is a key member of the Integrated Community Team, providing robust clinical services for patients, working closely with allocated GP practices across their Integrated Care Networks and with members of the team from other professions, putting the patient at the centre of their care.

The District Nurse/Senior Community Staff Nurse is responsible for the management of a defined caseload of patients and carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of care programmes. You will be accountable for the standards of care and for determining the activity of your team of support staff.

Your normal pattern of work will be agreed with you by your Line Manager subject to the needs of the Employer, flexible working arrangements (agreed by you and your Line Manager) and the Working Time Regulations. The rota will be pre-planned and will include various shift patterns that fall between the hours of 8 am - 22.30 pm examples of these shifts are:

8.15 am - 16.15 pm
10.45 am - 18.45 pm
14.30 am - 22.30 pm

Please note these shifts are subject to variation.

In addition you will be required to work a minimum of 1 weekend in 4 weeks (there is flexibility to work more should you wish).

Person Specification: You will be need to be a qualified Adult Nurse with current NMC registration and previous experience gained within District or Community Nursing.
